Become a DESMOND facilitator
Help people manage type 2 diabetes with confidence
DESMOND is a structured self-management education program for people living with type 2 diabetes and their support people.
Delivered as a one-day group program, DESMOND helps participants build the knowledge, skills and confidence to manage their diabetes and make informed decisions about their health.
Diabetes WA trains registered health professionals to become DESMOND facilitators and deliver the program in their communities.

About DESMOND
DESMOND has been delivered in Western Australia for more than 15 years, with program content and activities regularly reviewed and refreshed.
Rather than simply providing information, DESMOND facilitators use an interactive approach that encourages participants to explore their own experiences, consider their options and build confidence in managing diabetes.
During the program, participants explore topics including:
- making informed food choices
- managing blood glucose with confidence
- building skills and routines to reduce the risk of diabetes-related complications
- understanding diabetes treatment choices
- setting goals for the future
The importance of the facilitator role
At Diabetes WA, we believe a statewide network of skilled health professionals enables more Western Australians living with diabetes to self-manage their condition close to home supported by clinicians they already know and trust.

Who we need
DESMOND facilitator training is available to registered health professionals who have the capacity to deliver regular one-day group programs.
If you plan to deliver DESMOND as part of your existing employment, you’ll need to discuss the opportunity with your organisation.
Once accredited, facilitators are expected to deliver at least three DESMOND programs each year. More frequent program delivery is required while completing the Quality Development pathway.
Pathways
Completing the initial two-day training is the first step towards becoming an accredited DESMOND Educator.
Following training, you’ll take part in a supported Quality Development process that gives you opportunities to develop your facilitation skills, reflect on your practice and demonstrate that you can deliver DESMOND to the required standard.
DESMOND programs can be delivered as part of your existing role, with the support of your organisation, or as a subcontractor.
Diabetes WA provides mentoring and support throughout this process.
