Diabetes WA presented four innovative projects at the International Diabetes Federation – Western Pacific Region and Australasian Diabetes Congress in Melbourne on 18-21 August, 2026.

These projects showcase community co-design, workforce development and innovative models of care, and we were incredibly proud to share them with our colleagues across the conference. Here’s a brief overview:

Diabetes WA Connect demonstrates how timely access to specialist diabetes advice is supporting GPs across metropolitan, regional and rural Western Australia to deliver evidence-based diabetes care closer to home.

Evaluation shows high GP satisfaction, increased confidence and meaningful impacts on clinical decision-making.

Yarning for Change (DESY) – highlights a culturally grounded, co-designed training program that has equipped Aboriginal Health Practitioners with the knowledge, skills and resources to deliver diabetes self-management education in ways that are meaningful, flexible and responsive to community needs.

Community Co-design: Digital Resources for Medication Use shares how feedback from more than 780 Australians living with type 2 diabetes informed the development of person-centred digital resources to support confidence and informed decision-making around diabetes medications.

From Lived Experience to Animation (oral presentation) showcases how people from five culturally and linguistically diverse communities co-designed in-language diabetes medication resources, ensuring community voices shaped every stage from interviews and script development through to the final animations.
